As legal guardian of the above-named persons, I recognize that injuries can occur in activities involving height or motion, including but not limited to dance, theatre games, stage performance, etc. I consent to my child participating in any and all programs at Town Square Playhouse and I accept all risks associated with that participation. I release Town Square Playhouse and all employees from liability for any and all damages or injuries suffered by my child while under the instruction, supervision or control of Town Square Playhouse.
I confirm that my child is in good health and that I have medical insurance on my child and will provide coverage while he/she is enrolled. I hereby authorize simple first aid and consent to any x-ray, exam, and medical or surgical diagnosis that is deemed necessary in case of emergency. Additionally, I hereby agree to individually provide for all possible future medical expenses which may be incurred by my child as a result of any injury sustained while participating at or for Town Square Playhouse. I will notify Town Square Playhouse of any special medical conditions pertaining to my child including allergies.
Under Georgia law, there is no liability for an injury or death of an individual entering these premises if such injury or death results from the inherent risks of contracting COVID-19. You are assuming this risk by entering these premises.
I also give permission for photographs and videos of my child to be used in print or broadcast media as deemed appropriate for the promotion of any Town Square Playhouse activities.
